Earlier Initiation of ARV Therapy
نویسنده
چکیده
Earlier Initiation of ARV Therapy Over the past few years, several studies have been presented and published that indicate that earlier initiation of antiretroviral (ARV) therapy decreases morbidity and mortality amongst HIV+ patients [Kitahata M, et al. New Engl J Med 2009;360:1815-26; Emery S, et al. JID 2008;197:1133-1144]. A number of studies reported on specific mechanisms that could be contributing to the pathogenesis of untreated HIV/AIDS, and, therefore, lend credence to the emerging trend of earlier initiation of ARV therapy, and one study assessed benefits of ARV therapy in newly-infected patients.

Rethinking the “Pre” in Pre-Therapy Counseling: No Benefit of Additional Visits Prior to Therapy on Adherence or Viremia in Ugandans Initiating ARVs
BACKGROUND Many guidelines recommend adherence counseling prior to initiating antiretrovirals (ARVs), however the additional benefit of pre-therapy counseling visits on early adherence is not known. We sought to assess for a benefit of adherence counseling visits prior to ARV initiation versus adherence counseling during the early treatment period. METHODS We performed a secondary analysis of...

The goal of ARV therapy in immunocompromised HIV-infected individuals is immune reconstitution. However, an aberrant manifestation of this effect sometimes occurs. Immune reconstitution inflammatory syndrome (IRIS), also known as immune restoration disease, refers to a diseaseor pathogen-specific inflammatory response in HIV-infected patients that may be triggered after: Initiation or re-init...
متن کاملWhen to start: as soon as possible
Debate regarding ''When to start'' antiretroviral (ARV) therapy has raged since the introduction of zidovudine in 1987. Based on the entry criteria for the original Burroughs Wellcome (002) study, the field has been anchored to ''CD4 counts'' as the prime metric to indicate ARV treatment initiation for asymptomatic HIV-positive individuals. The pendulum has swung back and forth, based mostly on...

Seven hundred and thirty-five (29.8% male and 70.2% female) patients who consecutively attended three HIV clinics completed assessments prior to ARV initiation, 519 af...
متن کاملSelf-initiation of antiretroviral therapy in the developing world: the involvement of private pharmacies in an HIV program
BACKGROUND Self-initiation to antiretroviral treatment (ART) exposes the patient to the risk of drug toxicity, poor adherence to treatment, and escalates the development of drug resistance. OBJECTIVES To determine the sources of antiretroviral (ARV) drugs by unregistered human immunodeficiency virus (HIV)-infected patients and the extent of ARV self-medication. METHODS Simulated clients wer...
